Conducts and documents accurate clinical assessments. Identifies and prioritizes patient/family needs and incorporates into the plan of care. Develops, implements and evaluates the nursing plan of care. Modifies plan to meet established clinical outcomes. Identifies discharge planning needs of patient/family/significant other on admission, developing, implementing and coordinating discharge planning, independently or in collaboration with other staff, in order to ensure comprehensive discharge support. Follows standards algorithms, guidelines, decision trees, and protocols. Seeks validation for own clinical decisions and integrates input from other clinicians for decision making. Recognizes limits related to clinical knowledge and engages broader healthcare team as appropriate. Seeks assistance in approaching multidisciplinary consultation to enhance patient outcomes. Demonstrates professional behavior and customer service standards on a consistent basis. Preserves, protects and advocates the patient's/family's confidentiality, autonomy, dignity and rights. Aware of patient/family a satisfaction scores, and contributes to unit initiatives for improvement. Engages in therapeutic relationships with patients/families and Interacts in a compassionate, empathetic and humanistic manner. Engages regularly in hourly rounding. Manages patient symptoms and individualizes care requirements based on standards, protocols, and policies ( to include emotional and physical pain and required comfort measures). Delegates tasks/care to appropriate staff and ensures timely follow-up. Uses a familiar array of strategies in conjunction with available clinical and?human resources for problem solving.
