Job Description

Job Summary:

RN Clinical Documentation Coder reviews and collaboratively corrects clinical documentation to ensure that level of patient services provided is accurately and completely documented.


Performs ongoing medical record review using documentation improvement guidelines to evaluate overall quality and completeness of clinical documentation. Conducts follow-up reviews of clinical documentation to ensure issues discussed and clarified with clinical staff have been recorded in patient's chart.

Works with physicians and medical staff to improve clinical documentation for more accurate code assignments and a more accurate case mix.

Establishes and maintains a system to track and analyze outcomes of documentation improvement program. Prepares regular outcomes progress reports and presents as designated to administrative staff and committees.

Uses performance improvement methodologies and education strategies to develop formal training and improvement programs for staff concerning clinical documentation opportunities, coding and reimbursement issues. Provides ongoing updates as regulations change.

Facilitates and participates in regular meetings with Health Information Management (HIM), Care Management, and other staff to develop and implement strategies for improvement of clinical documentation and financial outcomes.

Works collaboratively with Coding staff to ensure appropriate DRG assignment and that reimbursement reflects the level of services rendered.

Identifies learning needs related to physician documentation in order to plan education for physicians, coding specialists, and clinical staff. Educates physicians and key healthcare providers regarding clinical documentation improvement and the need for accurate and complete documentation in the health record.

Responsibilities may vary over time. Required to perform other duties as required for the efficiency and effectiveness of the department and job role.



Required Credential(s):

Certified Clinical Documentation Prof. credentialed from the Association of Clinical Documentation Improvement Specialists

Registered Nurse credentialed from the Oklahoma Board of Nursing .

One or more of the following:

Certified Coding Specialist (CCS) credentialed from the American Health Information Management Association (AHIMA) .

Reg Health Info Admnstr credentialed from the American Health Information Management Association (AHIMA) .

Reg Health Info Tech credentialed from the American Health Information Management Association (AHIMA) .


Associate's Degree in Nursing Required Bachelor's Degree preferred/Registered Nurse with current license to practice in the state of Oklahoma. CCDS Required, and one or more of the following CCS, or RHIA, or RHIT/ Minimum of 3 years’ experience as an RN

Work Experience:

Minimum of 3 years experience as an RN required.
