In this high-profile role, the Director of Social Work Services and Operations provides oversight, guidance and support for the processes related to the development, and outcomes of social work services across all Landmark markets. This position will develop the strategy, policy and implementation plans needed to provide quality care and services to our patients. The Director of Social Work will oversee the effectiveness of the day to day operations of the social work team including medical and clinical social work interventions, referral processes, scheduling, documentation, interdisciplinary team collaboration, and development of community and provider relationships.
What you'll be doing:
Provide operational support for all Landmark market social work teams.
Assist with strategy, process development, and implementation of corporate initiatives
Attend and participate in contracted health plan IDT, Clinical Operations, JOC’s and other operational meetings as required
Uses data to identify opportunities for program development
Participate and oversee the social work role in weekly Landmark multi-disciplinary team rounds and communicate recommendations with the clinical team.
Consult with the clinical team to determine and address issues surrounding the social determinants of health as part of care plan
Conduct chart reviews to monitor performance of all Landmark market social work staff
Actively participates in the development and implementation of performance/quality improvement and patient safety activities with the Landmark Team.
Identify potential technical or patient care issues and work with Landmark social work staff to make recommendations to address and avoid future issues.
Communicates effectively with the multi-disciplinary team.
Represent Social Work on institutional/ practice area committees and quality support teams.
Responsible for the design, development, contracting, interviewing and training for market level social work programs.
Participates in mentorship and professional development of newly hired/contracted social workers
Working with leadership team establishes the annual goals, incentives, growth and program development initiatives.
Coaching and Development of clinical supervisors, or directly of IDT members in smaller markets
Serve as a resource to the entire Landmark care delivery team
Provide in-services/lectures to the Landmark multi-disciplinary team.
What we're looking for:
7+ years of social work experience with increasing scope and responsibility (MSW. field work not included)
5+ years of clinical and administrative leadership experience in a healthcare setting
LCSW (Licensed Clinical Social Worker) or equivalent with the ability to oversee Social Workers to obtain their hours for licensure.
Master’s Degree (MSW) in Social Work accredited by the Council of Social Work Education preferred
Advanced degrees (i.e. PhD, MBA) a plus
